This NDIS cleaning quote request template helps a participant or authorised contact send the facts a provider needs to assess an indoor household-cleaning enquiry. Replace every bracketed prompt, delete sections that do not apply and keep sensitive access details out until the provider confirms a safe channel. Sending the template is not a booking or funding approval.

Use the exact address in the private message body, not in a public post or review. If an authorised contact is writing, state their role rather than implying authority through a relationship label alone.

- What must happen before a booking is confirmed?
This template does not supply answers on the provider’s behalf.
