Print it or copy the relevant sections, mark only the indoor rooms to be assessed and rank essential work. Marked work remains a question until the provider accepts it in writing. This checklist excludes gardening, exterior work, repairs, handyman tasks, personal care, transport, meal preparation, dishes, organising, decluttering, packing, moving items, clinical cleaning, mould remediation, biohazard work and pest treatment.

A shared address does not turn the participant’s request into an unbounded whole-house service.
Ask the provider to quote this order. If the list is larger than the proposed visit, decide which items move to another visit rather than leaving the choice unstated.
Add the exact address, preferred days, frequency options, access arrangement, pets, product sensitivities, authorised contact and billing pathway using the NDIS cleaning scope worksheet. Then copy the necessary details into the quote request template.
Do not place portal passwords, bank details, reusable access codes or unrelated medical information on a room checklist.
The NDIA says funding may only be used for NDIS supports related to disability and in line with the participant’s plan.
