An NDIS cleaning complaint should go to the channel that can address the issue. A provider can handle its own service, worker, invoice and communication records. The NDIS Quality and Safeguards Commission handles concerns about the quality and safety of NDIS supports and services and provider or worker conduct. The NDIA handles plan, funding, portal and some payment matters.
Call 000 if someone is in immediate danger or urgent emergency help is needed. A standard complaint form is not an emergency response. Preserve evidence only where it is safe to do so.
Serious abuse, neglect, exploitation, violence, unsafe conduct or other misconduct may need prompt reporting to the appropriate authority. The Commission’s current complaint page explains its channels and the kinds of matters it receives.
A complaint can involve more than one owner. Send each body the facts relevant to its role.
For a non-urgent, routine issue, the participant may start with the provider and request an explanation or correction. The quality-concerns guide shows how to compare the event with the agreed task list.
The participant does not have to approach the worker or provider first if doing so feels unsafe, is inappropriate or could create further risk. A trusted person or advocate can help.
State who is making the complaint, the participant affected, the service address, visit date, provider or worker involved and what happened. Separate direct observations, documents and third-party reports. Include the impact on the participant and any continuing risk.
Attach only relevant evidence: the applicable task scope, agreement section, message, invoice line, service record or carefully bounded photo. Keep original files and note dates.
Possible requested outcomes include an explanation, correction of a service or invoice record, updated task instruction, changed communication method, access control, staff review, service review or a response under the provider’s complaints process.
Do not assume the outcome is guaranteed. Ask the complaint body to acknowledge receipt, provide a written response, explain the process, identify any information still needed and state how updates will be provided.
Share enough information to identify and assess the complaint, but remove unrelated plan details, other residents’ information, portal credentials and unnecessary medical history. Use secure channels for identity documents or sensitive evidence.
The participant can decide who may assist and what they may receive. Read the privacy and information-sharing guide and the NDIA’s consent information before using a representative.
The Commission says a person may make a complaint anonymously or ask for confidentiality. Those choices can limit what the Commission can investigate or what outcome information it can provide, so read the current explanation before choosing.
If the participant gives contact details, say how and when it is safe to communicate. Do not place sensitive complaint information in an ordinary voicemail if that creates risk.
The NDIS Code of Conduct covers respect for rights, privacy, safe and competent support, integrity, honesty and transparency, action on quality and safety concerns, and prevention of abuse and misconduct.
Refer to the part that matches the facts. Avoid presenting a suspected breach as a final legal finding; describe the conduct and let the appropriate body assess it.
Keep the complaint reference, date submitted, acknowledgement, contact person, questions, responses and agreed actions in one record. Note when the participant expects an update and what remains unresolved.
If the first response does not address the material facts, explain the gap and ask how to seek review or escalate under the applicable process.
Decide whether future visits should continue, pause or stop while the complaint is assessed. Remove or change access permissions if trust or safety is affected. Tell only authorised billing, support and household contacts what they need to know.
