NDIS Cleaning Change Request Template

This NDIS cleaning change request template records a proposed adjustment to an existing indoor service. Replace every bracketed prompt and delete sections that do not apply. A request is not accepted until the provider confirms the change and any effect on scope, timing, price, access or the agreement in writing.

Copy-ready subject line

Subject: Change requested for indoor cleaning at [service suburb] — [effective date or visit date]

Use a clear date and identify whether this is one visit, a temporary period or an ongoing change. Keep the exact address inside the private message rather than a public subject line.

Opening and authority

Hello, I am requesting a change to the cleaning arrangement for [participant or agreed identifier] at [exact service address].

I am [the participant / nominee / another authorised contact / a support person assisting at the participant’s direction]. My authority for this request covers [scope / scheduling / access / products / billing / another defined function].

If authority is uncertain, pause and confirm it. The NDIA’s consent guidance says permission can be defined by person, purpose and period.

Identify the current arrangement

Current agreement, quote or task-list version: [document and date].

Current service pattern: [frequency, duration or quantity and confirmed timing].

Current indoor priorities: [brief list].

The next affected visit is [date and booking status].

Do not rely on a description such as “the usual clean” when the provider needs to identify the exact instruction being changed.

Select the change type

State the requested replacement

Please replace [current instruction] with [proposed instruction] from [date] until [date or ongoing review point].

The participant’s essential indoor outcome is [outcome]. The new priority order is:

  1. [room and task]
  2. [room and task]
  3. [room and task]

Work removed from the scope is [details]. Work added for assessment is [details].

A new task is not accepted merely because it appears in the message.

Separate one-visit scheduling changes

For the visit on [date], I request [cancel / move / hold]. Suitable alternatives are [options]. Please confirm the status of later bookings.

Explain the factual reason or trigger

The reason relevant to delivery is [participant priority changed / task did not fit / timing changed / access changed / product instruction changed / budget decision changed / another factual trigger].

Relevant visit record or service evidence: [date or reference].

Share only what is needed to assess the request. A complete health history or plan is not the default explanation.

Update access and household instructions

Entry method from [date]: [attended / authorised building contact / another method requiring confirmation].

Old key, fob, code or permission to withdraw: [bounded description].

New authorised access contact: [name, role and verified channel].

Pets, shared-home or indoor-route change: [details].

Do not send a reusable code in the general request. Ask the provider for a secure handling method.

Update products and surfaces

Product, fragrance, residue or surface instruction to remove: [details].

New instruction to assess: [exact product or restriction, room and surface].

Person authorised to approve a substitution: [name and role].

No label guarantees universal safety. A new product or unfamiliar surface may require assessment before use.

Update contacts or billing routing

Contact whose role ends or changes: [name, function and date].

New contact and exact authority: [name, function, channel and review date].

Invoice recipient or management-pathway detail to update: [verified information].

A billing contact does not automatically control scope or access. Do not include portal passwords, banking credentials or unrelated plan information.

Ask the provider to explain consequences

Please confirm whether the change affects the quoted scope, service quantity, timing, provider capacity, price basis, any separately proposed amount or the service agreement.

If a new quote or written variation is required, please provide it before applying the change.

Use the quote-variation and price-change guide. Do not infer a new rate, fee or total from the request.

Request a clear confirmation

Please reply with:

  • whether the request is accepted, declined or needs assessment
  • the exact visit or effective date affected
  • the revised room, task or priority scope
  • any revised timing, quantity or price information
  • any access, product, contact or invoice-routing action
  • the updated document or version that controls the service

I will treat the current arrangement as unchanged until written confirmation says otherwise.

The NDIA says participants can give feedback, ask providers to change a support and choose how they communicate.
